3. Should a player be IR eligible only between the publication of the official NFL injury report (usually Wednesday) and the end of that week's NFL slate of games... or ANYTIME in the course of a season that the aforementioned player was listed as "doubtful/questionable/out"?

4. What about players going into a bye week? What injury report do we use in that case (the prior week's report?)?
